What is an ACO?


An ACO is a group of health care providers, who are voluntarily partnering to give coordinated high-quality care to their Medicare patients.


In an ACO, Primary Care Providers (PCPs) are responsible for coordinating care to ensure that patients get the right care at the right time, while preventing avoidable admissions and reducing the total cost of care. As part of their agreement with CMS, ACO providers are held accountable for the quality, cost, and experience of care of their Medicare Fee-for-Service patients.

As a partner provider with Responsive Care Solutions, you’ll join like-minded providers in giving coordinated, high-quality care to your unique patient population – whether they are homebound or in an assisted living or long-term care facility. You’ll have access to a robust suite of managed services and supports necessary to excel in value-based contracting without requiring additional time away from patients. Additionally, you’ll have the opportunity to share best practices and actively participate in organizational oversight.     Many ACO enablement organizations are backed by venture capital and have a primary goal of creating shareholder returns by keeping as much of the Shared Savings for themselves. Our mission is strengthening independent providers and improving care for the communities you serve. 

  • Focused on Frail Elderly Populations

    While most ACOs focus on the general fee-for-service (FFS) population, Responsive Care Solutions is just for providers who provide comprehensive primary care for the frail elderly, whether they are homebound, in an assisted living (ALF) or long-term care (LTC) facility. Your patients and practice model are not the same as the general Medicare population - your ACO shouldn't be either.
