What is an ACO?
An ACO is a group of health care providers, who are voluntarily partnering to give coordinated high-quality care to their Medicare patients.
In an ACO, Primary Care Providers (PCPs) are responsible for coordinating care to ensure that patients get the right care at the right time, while preventing avoidable admissions and reducing the total cost of care. As part of their agreement with CMS, ACO providers are held accountable for the quality, cost, and experience of care of their Medicare Fee-for-Service patients.
